Public health insurance for low-income people, jointly funded by the federal and state governments. It covers doctor visits, hospital care, prescriptions, pregnancy, and children's care. States run it under federal rules, and each state has its own name and details (California calls it Medi-Cal).

- Adults 19-64 (ACA expansion)≤ 138% FPLMaryland adopted the ACA Medicaid expansion (low-income adults 19-64 up to about 138% FPL). The state publishes a monthly dollar table by household size (not a single percent), effective February 1, 2026: $1,835 for a household of 1, $3,795 for a household of 4. Meeting the limit does not by itself mean you qualify — apply to be sure.
- Children 0-18 (via MCHP)≤ 322% FPLThe Maryland Children's Health Program (MCHP) covers children under 19 up to about 322% FPL. Official monthly dollar limits (effective Feb 1, 2026): $4,283 for a household of 1, $8,855 for a household of 4. Higher-income MCHP (MCHP Premium) charges a small monthly premium.
- Pregnancy≤ 264% FPLPregnancy Medicaid (in the household count, the pregnant person plus the expected child count as at least 2). Official monthly limit (effective Feb 1, 2026): $4,763 for a household of 2, $7,260 for a household of 4 (about 264% FPL). 🌟 Undocumented pregnant people are covered up to 250% FPL under the Healthy Babies Equity Act — see the immigrant rule above.
- Aged/Blind/Disabled (65+/ABD, non-MAGI)Aged 65+, blind, or disabled (non-MAGI): income-based, with no single fixed %FPL. Official limits (effective Feb 1, 2026): $350 monthly income and $2,500 assets for one person ($392 income / $3,000 assets for two). The Employed Individuals with Disabilities (EID) program has no income cap.

Maryland's Medicaid immigrant rules, grounded in the Maryland Department of Health (MDH) and Maryland Health Connection official pages, read in layers: (1) General rule — for full Medicaid or the Maryland Children's Health Program (MCHP), most noncitizens must have a qualified noncitizen status and wait five years after getting that status. (2) Children and pregnant people are exempt from the five-year wait (Maryland adopted the ICHIA option): MDH states that if you are pregnant, or under age 21 and lawfully present but not yet a qualified noncitizen, you may apply for full Medicaid and do not have to wait five years. (3) 🌟 Pregnant people of ANY immigration status, including undocumented: under the Healthy Babies Equity Act (effective July 1, 2023), noncitizen pregnant individuals of any immigration status can get full Medicaid with income up to 250% FPL — coverage runs through pregnancy and for four months after birth, and Medicaid may pay for care up to three months before you applied. (4) Emergency Medicaid, including childbirth, is available to non-qualified noncitizens who are otherwise eligible, regardless of status. (5) 🔴 An honest limit — undocumented children who are NOT pregnant and NOT lawfully present are NOT covered by full Maryland Medicaid or MCHP (only emergency services); Maryland has not extended full children's coverage to undocumented children the way California, Illinois, New York, or New Jersey have. (6) 🔴 A federal change is coming: effective October 1, 2026, the federal OBBBA (H.R.1) narrows Medicaid eligibility for some lawfully present noncitizen groups — that federal rule lives on the shared Medicaid program page and is not restated here. This is a changing area; verify your own situation with Maryland Health Connection or your local health department.

An eligibility decision is generally made within 45 days (up to about 90 days for disability-based cases). Medicaid may pay for care up to three months before you applied ("Medicaid may help pay for health care you had three months before you applied."). You can apply year-round.

✅ Regular Medicaid does NOT count in the public charge test — receiving it does not affect your green card or immigration application.
⚠️ The one exception: Medicaid that pays for long-term institutional care (a long-term stay in a nursing facility or mental-health institution at government expense) DOES count. Everyday doctor visits, hospital care, prescriptions, and home- and community-based care are not this exception.
➕ Because the current rule excludes all non-institutional Medicaid, Medicaid for children, pregnancy, and emergencies also does not count. In mixed-status families, eligible citizen or qualified children can safely get the care they qualify for.
Public charge is assessed only for people applying for an immigrant visa abroad, or applying for adjustment of status (a green card) inside the United States.
Many categories are exempt by law: refugees, asylees, VAWA self-petitioners, T and U visa applicants, Temporary Protected Status (TPS), Special Immigrant Juveniles (SIJ), Cuban/Haitian entrants, and others.
Public charge is generally not assessed when a green-card holder renews their card or naturalizes; a returning green-card holder is assessed only in limited cases (for example, an absence of more than 180 days).
