面向低收入者的公費醫療保險,由聯邦與州政府共同出資,涵蓋門診、住院、處方藥、孕產、兒童等醫療服務。由各州在聯邦規則下經辦,每州有自己的名稱和細則(加州稱 Medi-Cal)。
本州關鍵差異
與大多數人預期相反的一條,放在最前面:在華盛頓特區,「不符合 Medicaid 資格」本身就是另一個項目的服務對象。DHCF 對 Health Care Alliance 的官方定義逐字是「一個由本地出資的項目,專為不符合 Medicaid 資格的特區居民提供醫療援助」。多數人默認「不符合 Medicaid = 只能自費」;特區專門為這批人建了一個用本地稅款出資的項目。 本站在 DHCF 的 Alliance 頁面上,未查到關於移民身分的字句 —— 該頁列出的條件只有三條:特區居民、符合財務標準、且「沒有任何其它健康或醫療保險」。本站不替它寫身分口徑,也不重述聯邦規則。請直接向 DHCF 問清((202) 442-5988)。 ( 附帶說明:DC Health Link 上有一頁專講「因移民身分不符合多數項目的居民有哪些醫保選擇」,該頁對本站返回 403,打不開 —— 這是本站的取數限制,不是那一頁不存在。) 三項變更自 2025 年 10 月 1 日起已經生效(不是「即將」,是已經): ① 合併:「自 2025 年 10 月 1 日起,Health Care Alliance 與移民兒童項目(ICP)合併為一個項目」,現名 DC Health Care Alliance。 ② 26 歲及以上不再接受新申請。官方逐字:「自 2025 年 10 月 1 日起,Health Care Alliance 項目將不再把 26 歲或以上的新申請人納入項目」。 但「新申請人」是官方定義過的,別被上面那句嚇走:官方逐字寫明新申請人指「目前未參加本項目、且不在續期期間的人」,並解釋「新申請人是指目前沒有在領 Health Care Alliance、但想要申請的人」。→ 若你已經在保,或正處在續期期間,你不屬於「新申請人」。 ③ 21 歲及以上的收入上限從 215% FPL 降到 138% FPL。 沒有變的三件:0–20 歲的收入上限沒有變;沒有面談要求;認證期仍為每 12 個月一次。 ③ 會在你 12 個月一次的複核那天才作用到你身上 —— 「我現在還有保險」不代表這條與你無關。若你 21 歲以上、家庭收入在 138% 到 215% FPL 之間,請現在就去問 DHCF「我下次複核會怎麼樣、我還有什麼別的路」。 DHCF 把 Alliance 手冊出了中文、西班牙文、阿姆哈拉文、韓文、越南文版本。去要一份你看得懂的。 實務提醒:若家中成員身分情況不一,遞交前可先諮詢持牌移民律師或 DOJ 認可機構(許多免費或低收費)。 公共負擔:聯邦結論(含 2026-09-18 起的變化、法定豁免、以及境外簽證與口岸的口徑)見本頁項目級說明,本行不重複 —— 重複即第二真相源。個案請諮詢持牌移民律師。
